APPLICANT REQUESTS THAT:
A special order be accomplished and the order number be placed
on her certificate for award of the Air Force Achievement Medal
(AFAM) for the period of 15 November 1992 to 11 June 1996.

APPLICANT CONTENDS THAT:
She lost the orders pertaining to her AFAM. There is no record
of this decoration in the automated records management system
(ARMS).
In support of her request, the applicant submitted copies of
signed AF Form 2274, AFAM Award Certificates dated
23 April 1996, a copy of the AF Form 2274, First Oak Leaf
Cluster (1OLC) to the AFAM Award Certificate dated
6 December 2011, and documents extracted from the military
personnel database system (MilPDS).

STATEMENT OF FACTS:
The applicant enlisted in the Regular Air Force on 12 June 1992.
She was progressively promoted to the grade of Senior Airman,
(E-4) and was honorably discharged on 11 June 1996. She
accepted an Air Force commission on 9 July 2004.
The applicants 11 June 1996, DD Form 214, Certificate of
Release or Discharge from Active Duty does not reflect award of
an AFAM.

AIR FORCE EVALUATION:
1. AFPC/DPSID recommends denial. DPSID states that the
applicant has not exhausted all administrative avenues to obtain
the Special Order authorizing award of the Air Force Achievement
Medal.
2. The applicant provided her original Air Force Achievement
Medal certification/citation signed by the Commander, 59th
Medical Operations Group, for meritorious service from
15 November 1992 to 11 June 1996. The applicant should seek
administrative relief from the original approval authority, the
59th Medical Operations Group, for the Special Order before
appealing to the Air Force Board for Correction of Military
Record (AFBCMR). If the applicant feels an injustice still
exists after the original approval authority has rendered a
final decision, she may then appeal to the AFBCMR for relief.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the
applicant on 4 November 2011 for review and comment within 30
days (Exhibit D). To date, a response has not been received.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations.
2. The application was not timely filed; however, it is in the
interest of justice to excuse the failure to timely file.
3. Sufficient relevant evidence has been presented to demonstrate
the existence of error or injustice warranting corrective action.
The applicant has provided a copy of the Air Force Achievement
Medal (AFAM) Award Certificate as evidence she was awarded the
AFAM for meritorious service during the period 15 November 1992 to
11 June 1996. Although a special order could not be located,
given the evidence that the original certificate appears to be
authentic with no alterations, the Board found this document
sufficient to resolve this matter in favor of the applicant. As
such, we recommend the applicants records be corrected as
indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air Force
relating to APPLICANT be corrected to show that:
a. A special order be accomplished to reflect she was awarded
the Air Force Achievement Medal on 23 April 1996.
b. Her DD Form 214, Certificate of Release or Discharge from
Active Duty, issued in conjunction with her 11 June 1996, release
from active duty, be amended in Item 13, Decorations, Medals,
Badges, Citations, and Campaign Ribbons Awarded or Authorized, to
include the Air Force Achievement Medal.

APPLICANT CONTENDS THAT: In preparation for the Air Force Reduction in Force Boards, she reviewed her records and noticed that two of her AFAMs reflect First Oak Leaf Cluster. They may not be awarded Defense/Joint decorations. The JSCM will not be awarded for any period of service for which a Military Department medal is awarded.
